Gonadotrophin Deficiency: A deficiency in hormones like gonadotropins (LH, FSH) may occur due to Kallmann syndrome or damage to the hypothalamus or pituitary gland from tumors, radiation, or injury.
Signs and Symptoms
The primary sign of delayed puberty is the lack of the typical physical changes associated with puberty. In girls, this may include the absence of breast development or menstruation, while boys may not experience testicular enlargement or voice deepening.
How Is Delayed Puberty Diagnosed?
Diagnosis is done through a thorough evaluation, which may include:
Hormonal Testing: Measuring levels of LH, FSH, estrogen, testosterone, and other hormones.
Karyotyping: A genetic test to look for chromosomal conditions.
Imaging: Ultrasound or MRI to check for issues such as undescended testes in boys or pelvic abnormalities in girls.
Treatment for Delayed Puberty
Treatment varies based on the cause and age of the individual. In many cases, hormone therapy is used to trigger the physical changes of puberty.
In Boys: Testosterone therapy (injections) is commonly used to stimulate puberty, including penis enlargement, voice deepening, and muscle growth.
In Girls: Estrogen therapy begins at a low dose and is gradually increased to encourage breast development and the growth of adult body features. Progesterone is often added when menstruation begins to regulate the cycle.
