Signs of Diabetes in Men: Early Symptoms, ED and Tests

Common signs of diabetes in men explained by an endocrinologist at TOTALL Indore

The signs of diabetes in men include the usual symptoms of frequent urination, thirst, tiredness, hunger and blurry vision, along with signs that men are less likely to connect to blood sugar: erectile difficulty, lower sex drive, genital infections, loss of muscle strength and a growing belly. Because these changes are easy to blame on age, work stress or alcohol, many men are diagnosed only when a complication forces the issue.

This guide goes beyond our main article on the signs of diabetes. It explains what is different in men, why erectile dysfunction can be the first clue, how to tell diabetes apart from a prostate problem when you are passing urine often, how the picture changes in your 30s, 40s and after 50, and which tests are worth asking for. If you read only one section, make it the one on belly fat, because that is where Indian men are most often missed.

Key points

  • Erectile difficulty, low desire and low energy can be early signs of diabetes, and they can also warn of heart disease.
  • Passing urine often after 50 may be diabetes, an enlarged prostate, or both. A blood sugar test separates them.
  • A waist above about 90 cm in an Indian man raises diabetes risk even when weight on the scale looks normal.
  • A fasting sugar test alone can miss diabetes. Ask for a post-meal sugar and an HbA1c as well.

What this guide covers

The core signs of diabetes in men are the same as in women

High blood sugar behaves the same way in any body. Glucose that cannot enter the cells spills into the urine and drags water with it, which leads to more urine, thirst, tiredness, hunger, weight loss and blurry vision. Cuts heal slowly, and skin and gum infections become more frequent. Each of these is explained with its cause in our guide to the 8 classic signs of diabetes, so we will not repeat it here. Treat those as your baseline, then look at the signs of diabetes in men below, which are the ones men tend to overlook. If you are a woman reading for a family member, the equivalent guide is on signs of diabetes in women.

Signs of diabetes in men that are easy to miss

Erectile difficulty

Difficulty getting or keeping an erection is one of the most common problems in men with diabetes, and it often appears earlier in life than in men without diabetes. Years of high sugar damage the small blood vessels and nerves that an erection depends on. Because it is a sensitive topic, many men put up with it for a long time before saying anything. It is covered in more detail in its own section below.

Low desire, low energy and low testosterone

Men with type 2 diabetes are more likely to have low testosterone, and low testosterone in turn makes insulin resistance worse, so the two feed each other. The signs are reduced sexual desire, tiredness that sleep does not fix, low mood, loss of muscle, more fat around the waist and fewer morning erections. These overlap with diabetes itself, which is the reason both should be checked together.

Infections of the genitals and skin

Sugar in the urine and on the skin encourages yeast and bacteria. In men this can show up as redness, itching, soreness or a white discharge under the foreskin (balanitis), and it is more common in men who are not circumcised. Fungal infections in the groin (jock itch), between the toes (athlete’s foot) and in the nails are also common, as are boils and gum infections. One episode means little. Infections that keep returning, together with thirst or tiredness, should lead to a blood sugar test. If you already take diabetes tablets and develop genital irritation, tell your doctor, because one group of medicines called SGLT2 inhibitors can cause it.

Muscle loss, weakness and a growing belly

When insulin does not work well, the body struggles to build and keep muscle. Men may notice that they feel weaker, lift less than before, or tire on stairs even though they have gained weight. The weight goes mostly to the waist. Diabetes and belly fat reinforce each other, which is why a rising waistline deserves as much attention as any symptom.

Fertility

Diabetes can affect male fertility, through lower testosterone, reduced semen quality and, in some men, ejaculation problems such as semen going backward into the bladder. If you and your partner are trying for a child, and you have the signs described here, a blood sugar test belongs in the work-up.

Erectile dysfunction as an early warning

An erection needs healthy blood vessels, healthy nerves and normal hormone levels, and diabetes can harm all three. That is why erectile dysfunction is often among the first noticeable signs of diabetes in men, and why a man who develops it with no clear cause should have his blood sugar checked, whatever his age.

There is a second reason to take it seriously. The blood vessels of the penis are small, so they can show damage before the larger arteries of the heart do. In many men, erectile dysfunction appears a few years before a heart attack or chest pain. It is worth treating as a prompt to check blood pressure, cholesterol and blood sugar, not as a private embarrassment.

Please do not buy tablets for erectile dysfunction from a pharmacy or online without a diagnosis. These medicines are not safe for every man, particularly those taking nitrates for heart problems. A doctor can find the cause, treat it, and bring down your sugar at the same time, which often improves the problem. TOTALL also treats sexual dysfunction linked to diabetes and hormones.

Frequent urination: diabetes or prostate?

After 50, many men start passing urine more often, and a lot of them assume it is the prostate. Sometimes it is. But diabetes can cause the same complaint, and the two can be present together, so guessing is a mistake. The pattern usually gives a clue.

DiabetesEnlarged prostate (BPH)
Volume of urineLarge amounts each timeSmall amounts each time
StreamNormal forceWeak, slow, or stop-start
ThirstStrong and persistentUsually normal
Other signsTiredness, hunger, blurry vision, weight loss, infectionsHesitancy, feeling the bladder is not empty, dribbling after passing urine
Night-time urinationYes, with large volumesYes, with small volumes
How to confirmFasting and post-meal sugar, HbA1cExamination, urine test, ultrasound and PSA as advised by a doctor

Diabetes can also damage the nerves that control the bladder, which makes emptying incomplete and raises the chance of urinary infections. A man with both problems may be treated for the prostate for months while high sugar goes unchecked. If you are over 50 with urinary symptoms, ask for a blood sugar test at the first visit.

Signs of diabetes in men by age

Age groupWhat is most commonWhat to watch for
Teens and 20sType 1 diabetes, and early type 2 in men with obesity or a strong family historyRapid weight loss, intense thirst, tiredness (type 1). Dark neck patches, belly fat (type 2)
30sType 2 diabetes in men with belly fat, low activity, fatty liver or high blood pressureFatigue, low desire, erectile difficulty, repeated skin and groin infections, tingling in the feet
40sThe decade in which many Indian men are first diagnosedTiredness, weight around the waist, blurry vision, slow healing, snoring, erectile difficulty
50s and beyondDiabetes together with prostate and heart problemsUrinary changes, weak muscles, falls, repeated infections, numbness, wounds that heal slowly

Signs of diabetes in men in their 30s

Men in their 30s rarely expect diabetes, which is why diabetes symptoms in men this age are usually found late. Long working hours, irregular meals, little exercise and weight gain around the waist are the common background. The first signs of diabetes in men this age are usually vague: low energy, poorer concentration, reduced desire and an increase in infections. A man in his 30s who has any of these together with a waistline above 90 cm should have a blood sugar test as a routine step, not a worry.

Signs of diabetes in men in their 40s

This is the age when many men are first diagnosed, often by chance during a company health check or an unrelated visit. Others notice blurry vision, tingling in the feet, constant tiredness or slow healing. A man in his 40s with snoring, high blood pressure or fatty liver on an ultrasound has a high chance of having raised sugar and should be tested every year. Many Indian guidelines suggest starting at 30, or earlier with risk factors.

Signs of diabetes in men over 50

After 50, diabetes often arrives with company. Prostate symptoms, high blood pressure, high cholesterol and early heart disease are all common, which makes it harder to know which symptom belongs to which condition. Diabetes symptoms in men over 50 are often quieter than the classic signs: weaker muscles, falls, numb feet, repeated infections, or a wound that will not close. Because the picture is crowded, a yearly test for sugar, kidneys, heart and eyes is more reliable than waiting for symptoms.

Why Indian men are often missed

Many Indian men with type 2 diabetes do not look overweight by the usual standards. South Asians tend to carry more fat around the abdomen and in the liver at the same body weight, which drives insulin resistance even at a BMI that looks acceptable. Waist measurement tells you more than the weighing scale. A waist of about 90 cm or more in a man is considered a risk marker in Indians. Our guides on waist circumference and belly fat in Indians and BMI cutoffs for Indians explain how to measure and read these numbers.

Other reasons men are missed are cultural and practical. Men tend to put off check-ups until something hurts, so the signs of diabetes in men are often noticed late. They put symptoms down to stress or age, and are reluctant to talk about erectile difficulty. And as noted in our main guide, a fasting sugar test alone can miss diabetes in people whose sugar rises mainly after meals, so a normal fasting report does not close the question when symptoms persist.

Alcohol, smoking and sleep

Three habits are worth naming, because they change both risk and symptoms. Heavy alcohol use damages the pancreas and liver, adds calories and belly fat, and can hide the signs of low blood sugar in men on insulin or certain tablets, which is dangerous. Smoking raises the risk of developing type 2 diabetes and sharply increases the risk of complications in the heart, eyes, kidneys and feet. Poor sleep matters too. Loud snoring with daytime sleepiness can signal sleep apnoea, which is common in men with type 2 diabetes and worsens blood sugar control, so mention it to your doctor if it describes you.

Heart and complication risk in men with diabetes

Men with diabetes tend to develop heart disease earlier than men without it, and Indian men develop it earlier still. Chest pressure, breathlessness on exertion, unusual sweating, or pain spreading to the arm or jaw are emergencies and need a hospital, whatever your sugar level. Because diabetes can dull pain through nerve damage, some men feel only tiredness or breathlessness during a heart attack.

Beyond the heart, uncontrolled sugar over many years affects the eyes, kidneys, nerves and feet. We explain this in how diabetes affects the body. The good news is that early diagnosis and steady control prevent most of it.

Tests and who should take them

The tests that diagnose diabetes are the same for men and women: a fasting plasma glucose of 126 mg/dL or higher, a 2-hour glucose of 200 mg/dL or higher after a 75 g glucose drink, or an HbA1c of 6.5% or higher. The full table, including the prediabetes ranges, is in our main guide on the signs of diabetes and how they are diagnosed.

What is particular to men is the extra testing that makes sense when symptoms point that way. If you have erectile difficulty, low desire or unexplained tiredness, a morning blood test for testosterone is reasonable, together with blood sugar, because treating one without the other leaves the picture incomplete. If you have urinary symptoms after 50, your doctor may add a urine test, an ultrasound and sometimes a PSA. A urine test for protein (albumin) checks the kidneys early. A lipid profile and blood pressure check cover the heart.

Ask for a blood sugar test now, even without symptoms, if you are 30 or older, or younger with any of the following:

  • A waist of about 90 cm or more, or a BMI of 23 or higher.
  • A parent or sibling with diabetes.
  • High blood pressure, high cholesterol or fatty liver.
  • Erectile difficulty, low desire or unexplained fatigue.
  • Recurrent skin, groin or genital infections.
  • Loud snoring with daytime sleepiness.

If the result is normal, repeat it every one to three years, or yearly if you remain at higher risk. If it shows prediabetes, an annual test is the minimum, and the weight, activity and sleep changes you make now can still stop progression. Our guide on avoiding diabetes risk factors is a good starting point.

What to do next, and care in Indore

If several of these signs of diabetes in men sound like you, get a fasting glucose, a post-meal glucose and an HbA1c at a reliable laboratory, and do not change your diet beforehand. Take the report to an endocrinologist or diabetologist. The type of diabetes, your hormone levels and your heart risk all shape the right plan. Please do not rely on herbal powders, juices or supplements marketed as sugar cures, and never stop prescribed medicines without your doctor.

Go to a hospital the same day for chest discomfort, breathlessness, repeated vomiting, abdominal pain, fast deep breathing, confusion or marked drowsiness with high sugar.

At TOTALL Diabetes Hormone Institute in Indore, men can see an endocrinologist for diabetes and the hormonal problems that go with it, including sexual dysfunction, thyroid disease and obesity. Once diabetes is confirmed, TOTALL’s Diabetes Body Checkup is designed as an annual report card of the organs diabetes can affect, including the heart, kidneys, eyes, nerves and feet.

Frequently asked questions about the signs of diabetes in men

What are the early signs of diabetes in men?

Early signs in men include frequent urination, thirst, tiredness and blurry vision, along with erectile difficulty, lower sex drive, genital or groin infections and a growing belly. Many men have no early signs at all, so men with belly fat, high blood pressure or a family history should be tested without waiting for symptoms.

What are the first signs of diabetes in men?

The first signs are usually passing urine more often and feeling unusually thirsty or tired. In men, erectile difficulty and repeated skin or groin infections are also common first clues. They are easy to blame on stress or age, so a blood sugar test is the only reliable way to know.

Can erectile dysfunction be a sign of diabetes?

Yes. High blood sugar damages the small blood vessels and nerves that an erection depends on, and erectile difficulty is often one of the first noticeable signs. It can also warn of heart disease, so it is worth checking blood sugar, blood pressure and cholesterol together.

Is frequent urination in men diabetes or prostate?

It can be either, or both. Diabetes usually causes large volumes of urine with strong thirst, while an enlarged prostate causes a weak stream with small volumes and a feeling of incomplete emptying. A blood sugar test and a prostate assessment are needed to separate them.

Does diabetes lower testosterone in men?

Men with type 2 diabetes are more likely to have low testosterone, and low testosterone can make insulin resistance worse. Symptoms include low desire, fatigue, low mood and loss of muscle. If you have these signs, ask your doctor about a morning testosterone test along with blood sugar.

What are the signs of diabetes in men over 50?

After 50, signs are often quieter: urinary changes, weaker muscles, falls, numb or tingling feet, repeated infections and slow-healing wounds. Because prostate and heart problems are also common at this age, a yearly test for blood sugar, kidneys and heart is more reliable than waiting for symptoms.

Can a slim man have diabetes?

Yes. Indian men often carry fat around the abdomen and liver even at a normal weight, which causes insulin resistance. A waist of about 90 cm or more is a risk marker, and a normal BMI does not rule out diabetes.

Does diabetes cause infections in men?

It can. High sugar encourages yeast and bacteria, leading to redness and soreness under the foreskin, groin and foot fungal infections, boils and gum problems. A single episode is common, but infections that keep returning, especially with thirst or tiredness, should prompt a blood sugar test.

Does alcohol cause diabetes?

Heavy alcohol use raises the risk by adding calories and belly fat and by damaging the liver and pancreas. In men already on insulin or certain tablets, alcohol can also hide the warning signs of low blood sugar, which makes it risky.

Which tests should a man take if he suspects diabetes?

Start with a fasting plasma glucose, a post-meal glucose and an HbA1c. If you also have erectile difficulty or low desire, a morning testosterone test is reasonable. A urine albumin test, lipid profile and blood pressure check help assess kidney and heart risk.

Where can a man get tested for diabetes in Indore?

You can get blood sugar tests and consult an endocrinologist at TOTALL Diabetes Hormone Institute, BCM Health Island, Scheme No 54, Indore. It is open Monday to Saturday from 8:00 AM to 8:00 PM. Call 093024 43344, message on WhatsApp Book Appointment at TOTALL , or book an appointment online.
